PSTC — Pre-enrolment declaration

Part 2 of the Enrolment Application. Complete this after sending the Enrolment Application. The Registrar will not verify your application until this declaration is received and reviewed against the full period of the batch you chose (live sessions, self-paced work, assessments and on-site practical).

1. Full name as it appears on your SIRB (same as in your Enrolment Application)

2. Email address used in your Enrolment Application

3. Batch you applied for: its DL Code, or "next batch"

Answer for the full selected course period. If a batch is not yet scheduled, say “next batch” above and provide the information known now; the Registrar must reassess against the published timetable before verification. “Not sure” means no seat pending clarification. For details fields, write “None” or “Not applicable” only where the corresponding answer makes that truthful.

4. Is any travel, embarkation, other training or commitment scheduled to overlap any part of the selected batch’s full course period?

5. If Yes, identify commitment and start/end dates; if Not sure, explain the uncertainty; if No, enter None.

6. Will you be aboard a vessel during any part of this course period (including if already aboard before it starts)?

7. If Yes, state expected aboard start/end dates and expected course completion date; if Not sure, explain; if No, enter Not applicable.

If aboard, answer each feasibility question for the vessel and the selected batch. If not aboard, select “Not applicable — not aboard” for each. Do not use that option if your aboard status is uncertain.

8. Will vessel connectivity reliably support the LMS, media and mandatory live sessions?

9. Will protected off-duty time cover study and attendance throughout the course?

10. Can you participate without interfering with watches, safety-critical duties or vessel operations?

11. Can you attend all mandatory practicals and assessments at the required place and time?

12. I will promptly notify the Registrar if my travel, vessel roster, connectivity or course schedule changes.

I declare that the information above is true and complete. I understand that false information means my application is refused or my enrolment is cancelled.
